SUMMARY

The Coordinator of Support Living Services plans and coordinates quality control program designed to ensure continuous services consistent with established agency and state regulatory standards by performing the following duties. This person supervises and coordinates the day-to-day operation of several CRS sites while training leads/supervisors to take on responsibilities independently. They may also act as manager of more than one CRS site.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  • Ensures that program is adequately staffed as required. Works with the Lead/ Supervisors/Support staff to staff Sites.
  • Ensures that the program is operated within the mandated budget. Provides hands-on training to staff on standardized systems to ensure program quality.
  • Maintains systems for communicating important information to all program management.
  • Assists in hiring process for all staff.
  • Has on-call responsibilities for assigned programs, shared with leads on coordinator's days off Completes all provisional and annual staff evaluations Ensures the completion of all routine in-house reporting requirements (i.e. monthly reports, safety checklist, etc.)
  • Develops, implements and ensures staff implementation of resident teaching strategies and programs utilizing the IDT process. Implements a daily program schedule including daily living skills and community recreation.
  • Completes all quarterly and annual reviews in a timely fashion and submits the completed reports to the Case Manager and team 2 weeks before the annual IP meeting.
  • Assists in the development and writing of program goals.
  • Coordinates and runs all IDT meetings and ensures that all information is available including timely notices of meetings to all parties.
  • Provides supervision to all staff. Makes recommendations for hire and personnel action regarding staff to the Director.
  • Ensures that the program and administrative policies, procedures, and protocols are implemented.
  • Works with the business office to assure that individuals are receiving entitlements (i.e. SSI, Medicaid, DSS Subsidy, etc.)
  • Ensures that time cards, mileage reimbursement, grocery receipts, are submitted in an accurate, appropriate and timely manner.
  • Ensures resident finances/bank accounts and petty cash monies are properly maintained. This includes receipts and other proper documentation
  • Ensures that the home/sites, vehicle/s and agency equipment is kept clean, safe and in good working order including monthly vehicle checks and follow up.
  • Ensures communication meets expectations including voicemail use, e-mail use, meetings with subordinates, conversation with guardians and funding sources, etc. Ensure that the Individualized Plans are being followed.



EDUCATION and/or EXPERIENCE: a Bachelor's Degree in related field plus 5 years of progressive experience working in human services is required. High School diploma/GED with progressive experience in a Human Services field. At least one year of supervisory experience required as part of the previous experience.

CERTIFICATES, LICENSES, REGISTRATIONS



  • Valid Connecticut driver's license
  • Department of Developmental Services Medications Administration Certification.
  • PMT Certification
  • CPR/First Aid



SALARY



  • $21.07/hr, increases to 23.43/hr after completion of 6 month introductory period.
