POSITION TITLE: Case Manager

DEPARTMENT: Social Services

UNIT: Bremerton Corps

STATUS: Temporary, Full-Time, Hourly

REPORTS TO: Social Services Director

GENERAL STATEMENT:

SCOPE OF POSITION:

The Case Manager provides crisis response and case management to families and individuals in need who come to The Salvation Army in Bremerton for assistance. Duties include, but are not limited to, engaging with individuals to determine needs, providing emergency intervention, and finding resources as needed.

This role is a direct result of contractual funding and may be subject to change based on the scope and/or duration of the contract guidelines.

ESSENTIAL DUTIES AND RESPONSIBILITIES:



  • Maintaining social services office hours as assigned by the Director of Social Services
  • Providing social services case management for families and individuals
  • Providing housing resources to homeless households
  • Providing rental assistance to those exiting the shelter when funds are available
  • Providing case management for the food assistance, rental assistance, and/or utilities assistance programs
  • Providing case management for those experiencing addiction who want to move toward treatment
  • Keeping in contact with other agencies that can provide resources
  • Researching and providing current resources for social services staff, volunteers, and interns
  • Providing weekly, monthly, quarterly, or annual reports as assigned
  • Ensuring that daily data is submitted to HMIS/Wellsky as required
  • Maintaining appropriate records, a filing system, and a tidy workplace
  • Providing food assistance in cooperation with the Food Pantry Manager and the Warehouse Operations Manager
  • Assisting the Director of Social Services with administrative duties
  • Seasonal assignments (Angel Tree registration, Christmas food box and toy distribution)
  • Other duties as assigned by the Director of Social Services, or the corps officers
  • May be expected to work evenings and/or weekends if needed



EDUCATION AND WORK EXPERIENCE



  • Bachelor of Social Work (BSW), a BA/BS degree in a related field, or equivalent education or experience
  • 3 to 5 years of experience in social services
  • Knowledge of case management principles

  • Strong verbal and written communication skills

  • Self-motivated with the ability to work with minimal supervision
  • Ability to work in a challenging environment and maintain poise under pressure
  • Ability and willingness to keep information confidential
  • Must complete and adhere to Protecting the Mission requirements and guidelines, including the ability to pass a TSA background check
  • WA State Food Worker's Card must be attained within one month of employment
  • Willingness to serve a diverse population



SOFTWARE-REALTED SKILLS:



  • Microsoft Office, including Word, Excel, and Outlook (email)
  • Data Entry - able to learn skills required to enter clients.



PHYSICAL REQUIREMNTS:



  • Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis
  • Ability to operate telephone, and a desktop or laptop computer
  • Ability to lift up to 25 lbs. (emptying trash, moving boxes, etc.)
