Role: E6 Peer Case Manager With the support of and in partnership with the E6 team, the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals, and is responsible for identifying, engaging and providing support to homeless clients, including aftercare services and follow up support, once housed. The Case Manager will focus specifically in the Midtown Los Angeles area and will engage homeless individuals, as well as provide linkages to housing and supportive services. These teams are part of a large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Case Manager provides information and support for program members, and assists members in linking to a variety of resources and services as needed. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.
