Role: Clinical Case Manager
Reports to: Program Manager

Location: Westside Mental Health Office - 1749 14th St., Santa Monica, CA 90404
Schedule: Monday - Friday, 8:00am - 4:30pm
Status: Full Time, Non-exempt (Hourly), Non-management

SUMMARY

The Clinical Case Manager will provide field-based services to homeless individuals in Malibu and the Pacific palisades, link them with integrated services, and coordinate mental and recovery services to ensure housing stability and community integration. This individual will assist in general outreach with teams as well as maintain a caseload for ongoing care.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  • Assess mental health services needs of clients in the filed
  • Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan
  • Provide crisis intervention and clinical case management services to clients
  • Provide long-term case management, advocacy, and retention services to housed clients
  • Develop effective and professional relationships with property owners/managers and housing providers
  • Develop effective, trusting relationships with clients, with a focus on facilitating their independence and long term-housing stability
  • Meet with client as often as necessary to provide ongoing support and guidance based upon individual client needs; update goal plan(s) as changes occur, and help to ensure client's retention of permanent housing
  • Assist client with integration into the community in which they live; assess and provide life skill activities
  • Meet with client in field as much as possible; at minimum, provide quarterly home visits
  • Facilitate linkage to mental health, substance abuse and medical services, and vocational resources and provide transportation when needed
  • Communicate with mental health, substance abuse, and medical provider to ensure continuity of care
  • Assist clients in maintaining benefits and increasing benefits when needed
  • Attend and participate in case file, staff meetings, and required training
  • Provide outreach to housing providers in a timely manner to effectively resolve landlord/tenant issues
  • Organize documentation in client files and computer database, and complete all charting in a timely manner
  • Complete services statistics and reports as required by funders
  • Communicate effectively with housing authorities on behalf of clients



QUALIFICATIONS



  • Master's degree in a mental health field and a minimum of one years' case management experience
  • Able to write concise case notes
  • Outstanding recordkeeping skills
  • Able to maintain an effective working relationship with community partners and maintain and build professional rapport with staff
  • Able to read, interpret, and apply complex data collection guidelines and meet reporting deadlines
  • Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers
  • Demonstrated knowledge of issues faced by population served
  • Committed to improving the community through empowerment and dignity
  • Skilled in non-violent crisis intervention
  • Detail oriented with strong time management, organizational, planning, written, verbal, and interpersonal skills
  • Able to work as part of a team and be highly independent in achieving work duties
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record
