Responsibilities

The Transfer Center Nurse facilitates the transfer of all emergent, urgent and elective patients into UC Davis Medical Center from all referring locations. The Transfer Center Nurse demonstrates clinical expertise, quality assessment and excellent interdisciplinary communication to ensure the coordination and timely transfer of all requested patient transfers. The Transfer Center Nurse also facilitates direct admits from UC Davis Health Clinics and phone consultations and tele-medicine requests from other facilities by connecting the referring providers with the appropriate hospital service and physician in a timely manner.

The Transfer Center Nurse is responsible for evaluating, coordinating, processing, screening and documenting patient transfers into the UC Davis Health. The Transfer Center Nurse is responsible for obtaining appropriate medical and demographic patient information and coordinating the communication between the referring and the receiving providers, as well as between healthcare facilities. The Transfer Center Nurse will utilize problem solving and customer service skills to determine the best course of action for the patient, the physician, and the hospital by working closely with the UC Davis House Supervisor, the physician, the ED and the inpatient staff to ensure the effective and efficient transfer of every patient.

Required Qualifications

  • Possession of a current California RN license.
  • American Heart Association (AHA) CPR Certification
  • Graduation from an accredited School of Nursing, BSN preferred.
  • At least one (1) year of recent Case Management Experience
  • At least three (3) years of combined inpatient experience in: Medical surgical/ Emergency Department/ Intensive Care Unit
  • Licensed RN with current working knowledge of discharge planning, utilization management, performance improvement and managed care reimbursement.
  • Knowledge of Medicare, MediCal, and HMO legislation, MI county contracts, third party payor contracts, Title XXII, the Joint Commission, and Lumetra criteria. Demonstrated knowledge of how these programs/regulations affect the functions and role of the department. Demonstrates the ability to incorporate them into departmental systems/processes.
  • Knowledge of professional and ethical standards related to Clinical Case Management such as the Nursing Practice Ace, the Codes of Ethics from the American Nurses Association and the Standards of Practice & Scope of Services by the American Case Management Association.
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.

  • ED and/or ICU experience.
