The Ambulatory Care Social Worker I is a member of the care management team, under the auspices of the Director of Ambulatory Care Management and Ambulatory Care Supervisor. The Ambulatory Care Social Worker I (ACMSW 1) works in conjunction with Community Health Workers and works with all behavioral health matters for the Medical Management Department. Provides assistance with outpatient members, providers and caregivers to facilitate intensive, comprehensive care management and increase efficient utilization of support and community services. The Ambulatory Care Social Worker I will conduct escalated service coordination on social and social determinant complex cases and monitor through resolution, according to program guidelines. The Ambulatory Care Social Worker I is responsible for assessment and intervention to meet the social determinant needs of Care Management patients and their families/caregivers in the outpatient setting. This position includes advocacy, research, education, and any other assigned responsibilities relating to Care Management social work.

Primary Responsibilities:



  • Provides social work assessment and intervention to meet the social and social determinant needs of patients in care management and their families and significant others
  • Works with Community Health Workers.
  • Ensure compliance with program policy or projects assigned
  • Provide guidance for patients and the families, to understand and utilize services and education provided by the medical group and /or other agencies
  • Enhances the overall functioning and coping of the patient and family/caregivers within the community through community resource education
  • Responsible for social work referrals in collaboration with the interdisciplinary team
  • Provides oversight for social adjustment, supportive and crisis intervention information for care management patients and their families/caregivers
  • Maintains a comprehensive list of accurate and up-to-date list of agencies and referrals to ensure an adequate and comprehensive support process for the care management patient
  • Responsible for reviewing, screening and prioritizing cases for possible SW and care management services. Identifies cases that need to be expedited to the appropriate care for members with urgent or immediate needs
  • Other duties as assigned

Required Qualifications:



  • A bachelor's degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measures, or therapeutic techniques, such as social work, psychology, sociology, guidance and counseling, and child development
  • Licensed as a LCSW or LMFT
  • 1+ years of experience working in healthcare, or in providing social services.
  • Valid California driver's license required and insurability under the company's insurance carrier standards as applicable to job requirements.
  • Proficient computer skills and using Microsoft Office applications
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:



  • Master's degree in Social Work, Health Care Administration or Health Care field
  • 2+ years of experience in an IPA/Medical Group or health plan in a managed care environment
  • Bilingual skills desirable (Spanish)

California Residents Only: The salary range for California residents is $46,700 to $91,300.
