Job Summary and Qualifications

RN Case Manager
Case Management Department
Full Time, Day Shift, 1.0 FTE

The RN CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

  • Demonstrates responsibility and accountability in the performance of assessment, reassessment, development and implementation of the patient's plan of care in relationship to the discharge needs and follow-up care and in accordance with the utilization management plan.
  • Proactively communicates with the physician and other members of the health care team to determine the best options to meet the patient's needs in an efficient and effective manner.
  • Review all assigned cases within 1 work day of the admission or request for review utilizing current and accurate clinical case review criteria.
  • Promotes the most effective and efficient use of resources to achieve the targeted length of stay.
  • Assesses, collaborates and coordinates the patient's discharge planning needs and readiness for discharge with other members of the healthcare team and the patient/family/SO.
  • Complies with the departmental policies and procedures related to required processes including but not limited to documentation, interdisciplinary communication, IM delivery, teamwork, fiscal responsibility and performance improvement activities.
  • Assists Physicians in the decision of proper status placement. Informs and educates internal personnel for correct status assignment. Works with physician advisor to assure InterQual criteria are applied.
  • Refers to social worker the high-risk discharges e.g. homeless, HIV, DCF, Baker Acts, new diagnoses and difficult transfers/discharges/placements.

What qualifications you will need:
  • Associates degree required, BSN preferred
  • Hospital Nursing experience required
  • Case Management experience a plus
  • Critical Care experience a plus
  • Certification in Case Management or Utilization Review preferred
  • InterQual experience preferred
